Job descriptionProvide direct patient care under the supervision of RNs or physicians. Conduct health coaching sessions to promote wellness and chronic condition management. Perform accurate medical documentation in compliance with healthcare regulations. Assist in outpatient settings, LTC, and hospital medicine environments . Administer medications and treatments as prescribed. Monitor patients’ vital signs and report any changes. Support patients undergoing dialysis and educate them about the process. Utilize medical terminology to communicate effectively with healthcare teams. Conduct diagnostic evaluations to assess patients’ health status. Ensure infection control protocols and maintain a safe patient environment. Active LPN License in the state of practice in Massachusetts or a pending application for Massachusetts. Minimum 2 years of Long-Term Care (LTC) experience. Must be able to adapt quickly and work with minimal orientation. Experience working in memory care settings preferred. Excellent communication skills – verbal and written. Ability to work collaboratively while maintaining patient confidentiality. Flexibility to adapt to varied healthcare environments and schedules.
