Responsibilities

RN Case Managers are primarily responsible for providing professional nursing care to assigned patients with input from the IDT (interdisciplinary team). The RNCM evaluates, develops, implements the plan of care for each of their assigned patients.

Primary Service Locations: All of San Diego and south Riverside county.
Work Locations: Escondido, Mission Valley, Carlsbad, Temecula

Hours: Varies

On-Call: Rotation

Weekends: Rotation

Status: Full Time - benefits eligible

Salary: $40.41- $56.66 depending on experience

Qualifications

• Must have Associates Degree but a Bachelors Degree is preferred.
• At least 12 months experience with 1-2 years preferred in acute care
• Current CA RN license.
• Current CA driver’s license with proof of insurance.
• Computer skills (word processing), use of adding machine, calculator, fax, photocopier.
• Hospice experience preferred
• CPR/BLS current certification required.
• Current TB test results required.
