Priority Healthcare is seeking a dynamic Clinical Liaison to represent multiple skilled nursing facilities (SNFs) outside of the greater Harrisburg and York areas. This role is essential in driving referrals and admissions by fostering strong relationships with hospital case managers, social workers, physicians, and other key referral sources.

What We’re Looking For :

2+ years of experience as a Hospital Liaison, Clinical Liaison, or Director of missions in the skilled nursing or post-acute industry

Proven ability to increase admissions and census growth

Strong sales and relationship-building skills to engage hospitals, ALFs, senior centers, and other referral sources

Expertise in insurance guideline s and the financial criteria for SNF admissions

Ability to assess referrals to ensure proper clinical, equipment, and medication needs can be met

How You’ll Make an Impact :

Develop and maintain strong referral relationships with hospitals, clinics, rehab centers, and other healthcare organizations

Educate healthcare professionals on SNF services, post-acute care, and specialized programs

Strategize to increase census and enhance payor mix by identifying key market opportunities

Ensure smooth transitions from hospital to facility by collaborating with administrators, nursing, business office, and admissions teams

Participate in community outreach and networking events to expand referral partnerships
