Job description

Job Title : End-of-Life Care Specialist

As an End-of-Life Care Specialist, you will play a critical role in delivering high-quality care to patients and their families. Your expertise in nursing practice will enable you to navigate complex patient needs and provide compassionate support.

Key Responsibilities :

  • Deliver direct and indirect quality care to patients of all ages, prioritizing individualized needs.
  • Collaborate with the healthcare team to coordinate patient care activities and promote comfort.

Required Qualifications :

  • Current Oregon Registered Nurse License.
  • 1 year Medical / surgical experience, preferably in acute or long-term care settings.
  • Experience in EOL care is highly valued.

    • Preferred Qualifications :

  • Recent Oncology or Home Hospice or Home Health experience.
