Location : Neffsville Nursing and Rehab – Lancaster, PA

Neffsville Nursing and Rehab is seeking an experienced and compassionate Social Services Director to join our team. The Social Services Director serves as a liaison and advocate for residents’ social and emotional well-being, overseeing the planning, organization, and implementation of all social services within the facility.

Job Functions

Develop comprehensive social assessments and individualized care plans, identifying residents’ social and emotional needs, with measurable goals and action steps.

Participate actively in the pre-admission and admission processes to support residents and families during transitions.

Provide compassionate listening, counseling, and advocacy for residents and families, addressing issues of adjustment, crisis, or need.

Document residents’ behavioral and psychosocial status in accordance with regulatory standards.

Lead individual and group support efforts for residents and families.

Establish and oversee a volunteer program tailored to meet specific social service and engagement needs.

Orient residents and families to the facility, helping them adjust to their new environment and available resources.

Qualifications

Bachelor’s degree in Social Work or a related field from an accredited institution.

Knowledge of social services practices, procedures, regulations, and laws governing long-term care.

Excellent interpersonal, communication, and organizational skills.

Compassionate and professional demeanor when working with residents, families, and staff.

COVID-19 vaccination is required unless an approved exemption is provided upon hire.
