POSITION SUMMARY
To be fully engaged in providing Quality/No harm, Customer Experiences, and Stewardship by
performing exceptional treatments and providing appropriate age-specific therapy services under the
direction of a PT.
PRIMARY ACCOUNTABILITES
1. Adhere to all Health First, Inc., and Health First Home Care policies and procedures to maintain
compliance and care standards.
2. Treat patients according to the established plan of care, ensuring all interventions align with
treatment goals.
3. Address the plan of care as specified in the initial or re-evaluation, communicating any
necessary modifications to the Physical Therapist.
4. Ensure the safety of treatment procedures, patients, and the home environment at all times to
promote a secure care experience.
5. Comply with all relevant State and Federal regulations and JCAHO standards for Home Care
service delivery to meet quality and regulatory requirements.
6. Actively participate in Quality Improvement initiatives to support continuous enhancements in
patient care.
7. Accept assignments willingly, regardless of geographic territory, and attend at least 60% of staff
meetings to remain informed and engaged.
8. Assist in the organization of patient care, including scheduling and documentation reviews, as
requested by the supervisor.
9. Rotate weekend coverage as needed to assist with orthopedic pathway patients under the
direction of the Physical Therapist.
10. Inform patients and families of visit schedules in advance to maintain transparency and support
effective communication.
11. Ensure appropriate utilization of visits and services as directed by the Physical Therapist, and
document care accurately.
12. Utilize supplies in a cost-effective manner, meet productivity requirements, and aim to achieve
or exceed the 85th percentile in Press Ganey Patient Satisfaction.
Work Experience
MINIMUM QUALIFICATIONS
• Education: Graduate of an Physical Therapy Assisting program.
• Work Experience: Two (2) years of experience.
• Licensure:
o Current Licensure in the State of Florida as a Physical Therapy Assistant.
o Valid Driver's License and clean Motor Vehicle Record and maintained.
• Certification: American Heart Association (AHA) Basic Life Support (BLS) Healthcare Provider
Completion Card prior to start date and maintained.
• Background Check: Must satisfy Florida of Agency for Health Care Administration (AHCA)
Level 2 background screening.
• Skills/Knowledge/Abilities:
o Good communication skills.
o Ability to interact with other professionals, families, and patients.
o Ability to decide concern appropriate therapy and patient care.
o Ability to respond appropriately to life threatening situations.
o Ability to assess, interpret data and educate patients and families of various ages and
levels of understanding.
o Ability to work autonomously with time constraints and concentrate under mental
demands.
PREFERRED QUALIFICATIONS
• Work Experience: Previous Home Care experience.
PHYSICAL REQUIREMENTS
• Ability to move in crowded conditions.
• Ability to operate automobile in variable conditions.
• Visual acuity and hand-eye coordination to perform clinical tasks.
• Must be able to life, turn, pull, or ambulate with patients up to twenty (20) times daily.
• Must be able to lift up to forty (40) pounds unassisted up to six (6) time daily.
• Must be able to stand, walk, bend, and stoop for 90% of assigned shift.
• Must be able to tolerate a minimum of 3-5 hours of computer time a day.
