Job Summary
The Director of Case Management Services will play a pivotal role in implementing a comprehensive case management program. This includes overseeing Care Facilitation, Utilization Management, Case Management, and Discharge Planning.
The Director will develop systems and processes to enhance care / utilization management and discharge planning, and will also manage departmental activities related to clinical quality improvement. This position involves evaluating hospital resource utilization and reporting on financial and quality data linked to performance improvement. As a champion of interdisciplinary collaboration, the Director will foster teamwork and service excellence.
Minimum Qualifications
Education : Bachelor of Nursing (BSN) or Master of Social Work (MSW); Master's degree preferred.
Licenses / Certifications :
- Current and valid license to practice as a Registered Nurse in Texas or
- Current and valid Texas license as a Master's Social Worker (LMSW); LCSW preferred
- Certified Case Manager (CCM) or Accredited Case Manager (ACM) or
- Fellowship of the American Academy of Case Management (FAACM) required.
Experience / Knowledge / Skills :
Minimum of five (5) years in utilization management, case management, discharge planning, or related cost / quality management programs.Three (3) years of experience in hospital nursing or social work preferred.Five years of supervisory experience.Self-motivated with proven communication skills.Background in business planning and achieving targeted outcomes.Working knowledge of managed care and the health continuum.Familiarity with Performance Improvement concepts.Ability to build effective relationships with physicians.Collaborative attitude with healthcare professionals to enhance quality outcomes.Strong oral and written communication skills.Principal Accountabilities
Leads and supervises all aspects of the case management program.Promotes growth and development of the case management program aligned with enterprise-wide philosophy and health care trends.Manages day-to-day operational budget and ensures financial goals are met.Prepares and submits budget reports while forecasting expenses accurately.Takes corrective actions for any negative variances.Proposes capital budget items as necessary.Aims for optimal financial outcomes through effective case management processes.Oversees departmental personnel functions including hiring and performance appraisals.Facilitates communication among care partners such as physicians, nurses, and community care managers.Develops a high-performance team that embodies leadership principles in case management functions.Utilizes data to inform decision-making and implement performance improvement strategies.Educates various stakeholders about case management processes.Continuously identifies current and future customer needs.Ensures safety and adherence to policies, procedures, and standards.Encourages professional growth through mandatory education and mentorship.Demonstrates commitment to compassionate and personalized care experiences.Performs additional duties as assigned.
