Responsibilities

• Provide in-home physical therapy evaluations and treatments based on individualized patient care plans.
• Develop, implement, and revise care plans to help patients improve strength, mobility, and independence.
• Collaborate with physicians, nurses, and interdisciplinary team members to ensure coordinated patient care.
• Educate patients and caregivers on safe mobility techniques, exercises, and equipment use.
• Maintain accurate, timely documentation in compliance with agency policies and state / federal regulations.
