Position : RN Charge Nurse
Salary : $42-$55 / hr
Schedule : Monday, Tuesday, Wednesday AM 12-hour shifts
Shift differential for PM ($1.25 / hr.), NOC ($1.50 / hr.), and Weekend ($3.00 / hr.)

Manages and coordinates resident care from admission through discharge within SNF and occasionally, residential living; collaborates with other interdisciplinary health care team members including resident and responsible party; delegates responsibilities to nursing staff. Assesses residents identifies problems, plans, implements, and evaluates nursing interventions, reviewing and revising resident care plans as needed. Provides direct therapeutic, restorative, and preventive care to residents with identified health needs. Provides direction, supervision, and evaluation of unlicensed and licensed personnel in accordance with the policies and procedures of the facility.
Essential Job Duties
Admits, transfers, and discharges residents in coordination with other disciplines. Interacts with discharge planners. Requests needed information from hospitals and responsible parties for admissionsProvides leadership for the team through delegation of assigned duties and monitors all resident care and activities in the unitCoordinates daily assignments to ensure the delivery of cost-effective, quality resident careEnsures consistency of resident care through shift-to-shift rounds, every two-hour rounds, change of condition reports, and follow-up on doctor and family callsCoordinates nursing services with facility and community resourcesReceives, documents, and implements physician's ordersOrders and administers medications, performs treatments, schedules tests, and other diagnostic work as ordered by physiciansAssures that residents receive the prescribed diets. Reports resident weight variations and implements nutritional interventions as appropriate. Contracts dietician to establish appropriate dietary plans in response to resident weight variations in excess of 5% in one monthly periodMonitors and ensures completion of all medical record documentation, audits, and infection control logDocuments weekly assessments focusing on care plan goal achievements following the facility schedule. Reviews weekly assessments for completeness and accuracy. Initiates appropriate revisions. Documents significant occurrences in a timely mannerAll Employees must be able to pass background checks (fitness for duty physical, fingerprinting, employment references) as required by a licensed residential care facility.
