Schedule : Monday Friday 7 : 30 AM 4 : 00 PM plus occasional on call rotation

Location : Homestead, PA

Summary of Job Duties : If you are an RN with a background in home care or hospice and you have a passion for managing geriatric care-planning in a team environment, this might be the perfect fit for you. The RN Case Manager (RNCM) provides in-home assessments and overall care-management to a caseload of frail older adults whose goal is to continue living in their own home. RNCMs must be able to assess the need for, and coordinate the delivery of, nursing and home care services. This position involves local travel outside of the office, seeing patients for in-home assessments. The remainder of the day is spent visiting patients in our day center medical clinic, attending interdisciplinary meetings, intake & assessment care-planning meetings, and office time for documentation.

Required Education & Experience : Graduate of an accredited nursing school with at least one year of experience working with a frail elderly population, preferably in long-term care, home health care or community health setting. Experience teaching patients and other healthcare workers preferred.

Required Certifications and Licensure : Licensed Registered Nurse in Pennsylvania. Valid Drivers License.
