Job Description & Requirements Specialty : Physical Therapist Discipline : Therapy Start Date : 12 / 29 / 2025 Duration : 12 weeks 40 hours per week Shift : 8 hours, days Employment Type : Travel Shift Notes : 8 hr days - 40 hrs guaranteed Experience Required : 2 years PT experience Specialty : Long Term Care (LTC) Profession : PT Essential Functions and Responsibilities : Performs patient evaluations through the process of obtaining history, performing relevant systems review, and selecting and administering specific tests and measures to obtain data. Establishes an individualized plan of care and selects treatment techniques that addresses the specific impairments identified in the evaluation and in accordance with physician orders. Conducts ongoing reassessment of the patient and their response to treatment. Initiates and manages the discharge plan beginning at the first visit and continuing throughout treatment.
