Schedule : Full Time

Shift : Monday - Thursday 8am-5pm / Friday 8am - 12pm (sometimes until 5pm)

The RN Patient Care Coordinator is responsible for coordinating the care of patients within their specialty. Responsible for ensuring the full continuum of patient care is being met.
