Your mission : Deliver exceptional nursing care by performing patient assessments, administering medications, and providing treatments as outlined in the care plan. You will collaborate closely with registered nurses, physicians, and interdisciplinary teams to ensure seamless and personalized care for each patient. Key Responsibilities

Patient-Centered Care and Clinical Excellence

  • Patient Assessment : Perform comprehensive assessments during each visit, monitoring vital signs, wound care, and medication administration in accordance with care plans.
  • Care Plan Implementation : Deliver nursing care as outlined in the patient’s individualized care plan, ensuring accuracy, quality, and compliance with regulatory standards.
  • Patient Education : Provide patients and families with education on managing health conditions, proper medication use, and self-care strategies to promote independence and understanding.

Operational Excellence

  • Coordination of Care : Collaborate with registered nurses, physicians, and other care team members to ensure seamless care delivery and continuity of services.
  • Documentation : Maintain accurate and timely documentation of care activities, including assessments, medication administration, and changes in patient condition.
  • Performance Metrics : Ensure productivity by completing all assigned visits and meeting performance expectations while maintaining high-quality care delivery.
  • Regulatory Compliance and Safety

  • Compliance : Adhere to all state and federal regulations, HIPAA guidelines, and agency policies to ensure patient safety and confidentiality.
  • Safety Protocols : Follow infection control procedures, proper handling of medical supplies, and adherence to best practices to maintain a safe environment for patients and staff.
  • Collaboration and Team Support

  • Teamwork : Actively participate in case conferences, team meetings, and ongoing coordination to optimize patient outcomes and resolve care challenges.
  • Mentorship : Support and mentor other team members, including new hires, by sharing knowledge and expertise to foster a culture of collaboration.
  • Professional Development : Pursue ongoing education and training to stay updated on industry best practices, advances in healthcare, and changes in home health regulations.

  • Your Expertise

  • Education : Current and valid Licensed Practical Nurse (LPN) license issued by the state. Completion of an accredited nursing program.
  • Experience : At least one year of clinical nursing experience in a home health or healthcare setting, with knowledge of administering care in patients' homes.
  • Clinical Competency : Demonstrated ability to perform skilled nursing tasks, including medication administration, wound care, monitoring vital signs, and assisting with mobility and activities of daily living.
  • Regulatory Knowledge : Familiarity with state and federal home health regulations, including HIPAA and CMS’s Conditions of Participation, to ensure compliance in patient care.
  • Communication Skills : Excellent interpersonal and verbal communication skills to build trust and maintain effective relationships with patients, families, and interdisciplinary care teams.
  • Compassion and Empathy : A commitment to providing patient-centered, compassionate care that respects the dignity and independence of each patient.
  • Organizational Skills : Strong time management and prioritization skills to handle multiple patient visits and adapt to changing schedules effectively.
