This is a unique opportunity to make a lasting impact in the lives of older adults through interdisciplinary, person-centered care. What You'll Do : Provide evaluations, treatment, and care planning for older adults in the PACE center, homes, and other community settings Collaborate with an interdisciplinary team to promote functional independence and prevent hospitalizations Participate in team meetings, develop care plans, and support long-term quality of life goals for participants What We're Looking For : Current PA Occupational Therapist license At least 1 year of experience with geriatric or community-based populations preferred Flexibility, empathy, and strong communication skills Experience with home safety assessments and adaptive equipment a plus Why Choose MedAdventures?
