We can bring skilled care to you, provided by Home Health aides, nurses and therapists, to help you recover in the comfort of your own home. Should your needs evolve, we are here for you at the most critical of times, providing end-of-life care that allows you to live each day to its fullest.

The nurse plans organized and directed services and is experienced in nursing, with emphasis on community health education / experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of the individuals and families within their homes and communities.

Schedule : Full-Time, 32-40 hours per week guaranteed hours!

Salary : Starting at $38 / hour

Essential job functions and responsibilities :

  • License to Practice as an LPN under the direction of the RN case manager
  • Communicates changes in condition to RN case manager and / or physician
  • Participates in care coordination such as but not limited to patient / family meetings, goals of care meetings, interdisciplinary discipline team meetings, QAPI etc.
  • Arrange / Order any equipment, supplies related to patient care needs
  • Request refill / reorder medications from pharmacy and / or physician

Communication :

  • Prepare clinical notes and updates the primary physician when necessary
  • Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains physician's orders as required
  • Communicate with interdisciplinary team
  • Position Qualifications :

  • Graduate of an accredited school of nursing
  • At least one year of hands-on clinical experiences as a nurse (in any field of nursing) Hospice experience preferred but not required
  • Current license in state and CPR certification.
  • Excellent observation, verbal and written communication skills, problem solving skills, basic math skills; nursing skills per competency checklist.
  • Prolonged or considerable walking or standing. Able to lift, position or transfer patients. Able to lift supplies and equipment. Considerable reaching, stooping, bending, kneeling or crouching. Visual acuity and hearing to perform required nursing skills.
  • Must be a licensed driver with an automobile that is insured in accordance with state / or organization requirements and is in good working order.
