Job Description

Job Title : RN Care Manager – Direct Hire (Days)

  • Address : 20 Davis Ave, White Plains, NY 10601
  • Department : Case Management
  • Position Type : Direct Hire, Per Diem
  • Shift : Days (9 : 00 AM – 5 : 00 PM)
  • Weekend Requirement : Minimum 2 shifts per month
  • Guaranteed Hours : 24 hours / week
  • Salary Range : $60 / hr – $76 / hr
  • Benefits : Full suite of benefits + retirement plan

What We’re Looking For

  • BSN required
  • Active NYS RN license
  • PRI certification (or able to obtain within 3 months)
  • Strong computer and EMR skills
  • 3+ years in ED, critical care, or med-surg
  • Prior case management experience
  • Screening certification
  • Care Management certification (CMC, CCM, etc.)
  • Working knowledge of payer / regulatory guidelines and Milliman / InterQual criteria

    • What You’ll Do

  • Complete comprehensive initial assessments and ongoing reassessments throughout hospitalization
  • Coordinate multidisciplinary care across hospital levels
  • Lead discharge planning, including patient / caregiver education, aftercare arrangements, payer communication, and high-utilizer interventions
  • Perform Utilization Review (UR) duties including payer reviews, admission reviews, and clinical documentation using Milliman / InterQual criteria
