Job descriptionProvides, coordinates, and directs the provision of home nursing care according to physicians' orders based on agency policies and procedures through the competent application of the nursing process. Responsible to the Clinical Manager. Directly supervises and evaluates Home Health Aides and Licensed Practical Nurses / Licensed Vocational Nurses. Requires travel for home visits and periodic office meetings. Must be willing to travel up to 75 miles from home clinic; mileage reimbursed at IRS Standard rate. Assigned between 5-10 patients per day, equating to roughly 6-8 points based on visit types and activities (e.g., documentation, travel). Types of cases include Start of Care, Wound Care, Follow-up Visits, etc. Must be proficient in HomeCare HomeBase EMR and Oasis; if not used within the last 6 months, a retraining course is required. Responsible for visiting patients in both care facilities and patients' homes. Based on performance, potential for hire at the end of contract assignment. Productivity expectations are 28-30 units per week, with travel expected to be around 30-40 miles a day or approximately 200 miles weekly. On-call requirements and weekend work vary. Completion of documentation within 24 hours of patient appointments is mandatory. Flexibility to float to nearby clinics if assigned patient load does not meet financial needs.
