Job Description

The ideal candidate is a skilled professional that is passionate about delivering quality care and making a real difference with 1 year of experience in a Home Health setting.

Responsibilities

  • Assess each patient and develop an appropriate treatment plan with measurable goals and time frames
  • Prepare and submit clinical and progress summaries based on identified and attained goals
  • Ensure that other treating medical professionals have the necessary information to appropriately evaluate the patient
  • Experience with Oasis and Homecare Homebase a plus
  • Pay is $34.50 an hour with a nontax weekly stipend of $1631 per week

Qualifications

  • Bachelor's, Master's or Doctor's degree of an accredited physical therapy curriculum
  • Certification in Physical Therapy and valid CA state license to practice
