Position Summary
The Social Worker provides professional social services to individuals, families, and groups to help them cope with emotional, behavioral, social, and environmental challenges. Working in accordance with Connecticut Department of Public Health standards and the National Association of Social Workers (NASW) Code of Ethics , the Social Worker delivers compassionate, evidence-based care that promotes well-being, independence, and access to community resources.
This role requires empathy, excellent communication, and strong organizational skills to effectively support clients across healthcare, behavioral health, and community settings.
Key Responsibilities
- Conduct psychosocial assessments to evaluate clients' needs, strengths, and barriers to care.
- Develop and implement individualized care or treatment plans in collaboration with clients, families, and interdisciplinary teams.
- Provide individual, family, and group counseling to address emotional and social challenges.
- Offer crisis intervention, conflict resolution, and safety planning as needed.
- Coordinate referrals to community agencies , support services, and healthcare providers.
- Advocate for clients' rights and access to resources within healthcare and social systems.
- Maintain accurate and confidential case records in compliance with HIPAA and organizational policies.
- Participate in case reviews, care conferences, and interdisciplinary team meetings .
- Educate clients and families about available community resources, programs, and coping strategies .
- Adhere to professional, legal, and ethical standards for social work practice in Connecticut.
Qualifications
Bachelor's or Master's Degree in Social Work (BSW or MSW) from an accredited program.Active Connecticut Social Work License (LMSW, LCSW, or appropriate credential per scope of practice).Minimum 1-2 years of experience in social work, case management, or counseling (internship experience may be considered).Current CPR / BLS certification preferred.Strong assessment, communication, and documentation skills.Ability to work independently and collaboratively in a team environment.Must be available at least 3 days per week , with flexibility for evenings or weekends as needed.Preferred Skills
