Details below! Billing Note : The selected providers will be assigned to the St Anthony job in Gig Harbor for Friday shifts which will be billed separately under its own cost location CL. This position will be formally opened once a provider is selected. RATE CAPS FOR SUBMISSION : Malpractice Must be included in rates. Orientation 300 - half the hourly / callback rate kicks in after 8 hours. Per hour oforientation or training beyond 8 hours Hourly - 600 - Per hour no OTHoliday Hourly - 900 - Per hour on holiday no OTOn Call Daily 4100 - Per 24 hours to include 4 hours gratis24 Hour Holiday Call Rate - 6150 - Per 24 hours to include 4 hours gratisCall Back 600 Per hour ofpatient contact hours after gratis hours are completed gratis prorate to MSAterms.
