Join a dedicated home health team providing compassionate, one-on-one care to patients across Washington State. We are looking for passionate Licensed Practical Nurses (LPNs) eager to grow their careers and make a meaningful impact in the community.

Qualifications

  • Active LPN license in Washington or a Compact License (required)
  • Home health experience preferred but not required
  • Strong attention to detail and excellent time management skills
  • Effective problem-solving and conflict resolution abilities
  • Good organizational and communication skills
  • Valid driver's license and reliable transportation

    • Roles & Responsibilities

  • Provide skilled nursing care following the active Plan of Care prescribed by physicians or healthcare professionals
  • Administer medications, treatments, and therapies accurately and compassionately
  • Maintain thorough and accurate documentation of nursing assessments, actions, patient responses, and care coordination
  • Educate patients and families on health conditions, disease management, prevention, and self-care techniques
  • Collaborate with interdisciplinary teams to ensure continuity and quality of care
  • Support patients with individualized 1 : 1 caseloads, providing personalized attention and care

    • Requirements

  • Flexibility to work full-time or part-time schedules based on patient needs
  • Ability to manage a 1 : 1 caseload with a focus on medically fragile patients
  • Commitment to ongoing training and professional growth
  • Willingness to travel within the designated counties : Skagit, King, Snohomish, Kitsap, Pierce, Thurston, Grays Harbor
  • Strong commitment to patient-centered, compassionate care
