Job Summary
Under general supervision of the department Director / Manager, supervises and oversees the operations of assigned staff and department responsibilities. Manages and monitors data collection and reports. Monitors compliance with Federal and State regulations governing the delivery of health care. Provides supervision for department and staff, coordinates work schedules, completes performance evaluations and recommends merit increases, participates in the selection of department staff, and assists in policy / procedure formation and program development, and participates in meeting department education needs. Competently covers vacant staff assignments as a working Supervisor.

Participate in committees regarding Hospital Case Management and Transitions of Care. Will participate on any other committees as directed by the HCM Director or Manager.
Available to provide consultation, education, direction, resource identification, problem resolution, seeking input and approval from Director / Manager, Hospital Case Management, when necessary.
Monitors data collection, audits and statistics for reporting as directed by the HCM Director or Manager.
Provides day-to-day supervision of assigned Hospital Case Management staff to assure effective patient through put and adherence to all policies, procedures, rules, and regulations.
Participates in hiring and training of staff for Hospital Management staff.
Creates Hospital Care Management staff assignments and monitors caseloads to optimal effectiveness and efficiency while continually making changes as indicated.
Performs audits of staff performance for completeness, accuracy, and clarity of documentation submitted by Hospital Care Management (HCM) staff per department chart audit policy and procedure. Seeks corrections, completion and / or clarification from HCM staff, providing education / instruction and reinforcement of procedures when applicable.
Resolves problems and mediates conflicts encountered during daily operations, determines appropriate solutions, and promotes teamwork. Encourages regular communication and informs staff of relevant business issues and their impact on the organization.
Competently covers vacant staff assignments as a working Supervisor.
Performs other duties as assigned.
Supervisory / Management Responsibility
This is a supervisor job which may have direct supervision of team members which may include hire / termination authority, disciplinary authority, and performance management responsibilities. May have budget input or responsibilities. Job is not considered a member of management staff.
Minimum Requirements
Education - Bachelor's degree in Nursing
Experience - Three (3) years of acute care RN experience including one (1) year of acute care case management.

Required Certifications, Registrations, Licenses
Currently licensed to practice as a Registered Nurse in South Carolina.
Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification is preferred.
