Job description

Join a mission-driven healthcare program in Erie, PA that helps older adults remain independent, healthy, and connected in their own homes and communities.

You’ll provide comprehensive nursing care in a collaborative environment, with most of your work taking place in a modern, well-equipped clinic and occasional visits to participants’ homes.

What You’ll Do

  • Deliver direct and indirect nursing care according to personalized care plans.
  • Assess patient needs, review diagnostic results, and take action on appropriate interventions.
  • Collaborate with a multidisciplinary team to update and adjust care plans
  • Document patient care and communicate updates to ensure smooth care transitions
  • Conduct occasional home visits and complete required EMR documentation
  • Contribute to quality improvement initiatives and support overall program goals

    • What You’ll Need

  • Graduate of an accredited nursing program with a current Pennsylvania RN license
  • ICU, ER, or Urgent Care experience preferred
  • 1+ year of experience with elderly or medically complex populations preferred
  • Strong critical thinking and decision-making skills
  • Excellent communication and interpersonal abilities
  • Ability to work independently and manage multiple priorities
