RN Case Manager

Job Title : RN Case Manager Location : Santa Rosa, California, 95405 Contract duration : 13 weeks Shift : 5X8 -hours Schedule : Days Key Responsibilities :

• Assess patients for case management needs and eligibility for services.
• Develop, implement, and evaluate individualized treatment and discharge plans.
• Coordinate interdisciplinary team meetings and family meetings to support patient care goals.
• Collaborate with physicians, nurses, social workers, and other care providers.
• Ensure compliance with CMS, NCQA, and The Joint Commission standards.
• Coordinate with insurance providers, HMOs, and IPAs for approvals and authorizations.
• Arrange post-acute services including DME, SNF, home health, and transportation.
• Educate patients and families regarding care plans, resources, and follow-up care.
• Advocate for patient needs while ensuring cost-effective care management.
• Accurately document all assessments, interventions, and outcomes in Epic.
• Monitor patient progress and update plans of care as appropriate. Requirements :
• Minimum 2 years of nursing experience
• Previous Charge Experience : Required
• Travel Experience : Required
• Charting System : Epic (required)
• Community Hospital Experience : Preferred
• LTAC Experience : Preferred
• Trauma Level I or II Experience : Preferred
• Certifications : BLS (required)
