Family Nurse Practitioner (FNP) – Outpatient Primary Care
- Location : Jackson Heights NY
- Schedule : Monday–Friday, 9 : 00am–6 : 00pm (30-minute unpaid lunch)
- Compensation : $135,000–$160,000 annually (based on experience)
An established community health organization in Manhattan is seeking a Family Nurse Practitioner (FNP) to join their outpatient primary care team. This role focuses on providing comprehensive, culturally competent care to diverse patient populations, including women’s health, the LGBTQIA+ community, and individuals living with or affected by HIV / AIDS. The ideal candidate is passionate about health equity, has experience working with underserved or vulnerable populations, and thrives in a collaborative, mission-driven environment. Please note : PA-C candidates will not be considered for this position.
Responsibilities include :
Provide comprehensive primary care to a panel of adult patientsEvaluate and manage acute and chronic medical conditionsPerform initial assessment and management of mental health conditions (e.g., depression, anxiety)Order, interpret, and act on diagnostic tests using evidence-based practicesPrescribe appropriate treatments and medications within the NP scope of practiceCounsel patients on treatment plans, preventive screenings, and lifestyle modificationsRefer patients to specialists as neededMaintain accurate and timely documentation within the EMR systemCollaborate with a multidisciplinary care team to ensure coordinated patient careParticipate in clinic, provider, and team meetingsContribute to quality improvement and peer review initiativesEngage in health promotion and community outreach activitiesParticipate in shared night and weekend call coverageQualifications :
Master’s Degree (MSN) or Doctoral Degree (DNP) in NursingCompletion of an accredited Nurse Practitioner programCertification as a Family Nurse Practitioner or Adult Nurse PractitionerCurrent and valid NYS NP license and DEA certificationMinimum of 2 years’ hands-on experience as an NPCurrent BLS and infection control certificationExcellent bedside manner and communication skillsProficiency in Spanish or an Asian language preferredAAHIVM and / or LGBT health certification a plus#zip
