Nurse Case Manager II – Field-Based (Illinois) Pay Rate : $35.87 / hour Employment Type : Full-Time, Contract Schedule : Monday – Friday, 8 : 00 AM – 5 : 00 PM CST Location : Illinois (Field-based; must reside in Kankakee County or nearby area) Position Overview We are seeking an experienced Nurse Case Manager II to provide field-based case management services to elderly individuals and people with disabilities who are approved for in-home or nursing facility care. This position is ideal for a compassionate, organized, and independent healthcare professional who thrives in both home-visit and remote work environments. The Case Manager will conduct in-home assessments, coordinate care, and advocate for members to ensure optimal, cost-effective outcomes through collaboration and comprehensive service planning. Key Responsibilities Conduct comprehensive assessments of members’ needs, eligibility, and benefits through clinical tools and data review. Develop and implement individualized case management plans to address member needs. Collaborate with healthcare providers, social workers, and community resources to coordinate care and services. Apply clinical judgment and established criteria to ensure proper administration of benefits and compliance with all regulatory standards. Utilize case management and quality management processes in accordance with company policies, accreditation standards, and state / federal regulations. Maintain accurate documentation in compliance with organizational and regulatory requirements. Coverage Area Primary County : Kankakee County, Illinois May include surrounding areas based on member needs. Requires travel to members’ homes approximately 50–75% of the time. Work from home when not in the field. Candidates must reside within or near the coverage area due to travel requirements. Required Qualifications Active, unrestricted Registered Nurse (RN), Licensed Clinical Social Worker (LCSW), or Licensed Clinical Professional Counselor (LCPC) license in the state of Illinois. Valid, unrestricted driver’s license and reliable vehicle. Residency in Illinois is required. At least 2–3 years of care management, discharge planning, or home health coordination experience. Minimum 3–5 years of clinical practice experience preferred. Strong communication, organizational, and time management skills. Proven ability to work independently with minimal supervision. Proficient computer and documentation skills, including navigating multiple systems and using Microsoft Word, Excel, Outlook, and PowerPoint. Preferred Qualifications Experience with Illinois waiver services or case management programs. Familiarity with managed care or field-based healthcare coordination. Excellent analytical, problem-solving, and interpersonal skills. Confidence working independently and collaborating virtually with cross-functional teams. Education Graduate of an accredited nursing or social work program. Current, unrestricted Illinois licensure as RN, LCSW, or LCPC (required). Additional Details Full-time, 40 hours per week. Field travel required (50–75%). Training and regular work hours : Monday through Friday, 8 : 00 AM – 5 : 00 PM CST.
