Bachelor of Science in Nursing (BSN) degree, active Nurse Practitioner (NP) license and minimum 1+ years Nurse Practitioner (NP) experience required. Applicants who do not meet these qualifications will not be considered.

There is an open Part Time Nurse Practitioner (NP) Home Health Flexible Schedule job in Oelwein, IA. The Part Time Nurse Practitioner (NP) will conduct home health risk assessments within Fayette County, IA. The average home health risk assessment will take about 45 minutes and the nature of the visit may vary and could include an annual health assessment or post discharge visit. The home health risk assessment will be conducted on Medicare Advantage plan members residing in Oelwein, IAand the surrounding Fayette County, IA.Fayette County, IA includes locations like Clermont, Fairbank, Hawkeye, Maynard, Oelwein, Randalia, St. Lucas, Stanley, Wadena, Waucoma, and Westgate.Candidates could be scheduled in any one of these locations during an individual shift.

Responsibilities :

• Conduct annual home health risk assessments for a Medicare Advantage health plan
• Evaluate prior and current medical history, medical conditions and medication history
• Provide education on medical conditions and medications
• Identify any gaps in care and provide recommendations for resolution
• No creation of treatment plans or prescribing of medications is required
