The Masters Social Worker provides skilled clinical and supportive interventions to patients and families, assisting them in coping with social, emotional, and financial challenges related to illness, hospitalization, and life transitions. This role involves case management, advocacy, crisis intervention, discharge planning, and interdisciplinary collaboration to ensure optimal patient outcomes and continuity of care.
Key Responsibilities :
Conduct timely psychosocial assessments for patients and families referred to social work services.
Provide professional support in areas such as crisis management, problem-solving, decision-making, and coping with disease, disability, or life-altering events.
Develop individualized care plans and integrate social work strategies into the interdisciplinary care process.
Deliver comprehensive case management, discharge planning, and continuing care services.
Facilitate insurance coverage and referrals for hospital or community-based services.
Address abuse / neglect cases, guardianship, adoption, psychiatric issues, and end-of-life concerns.
Educate patients and families about available community resources, healthcare options, and support systems.
Maintain detailed and accurate documentation in the electronic medical record (EMR).
Participate in community education and patient support programs as assigned.
Engage in continuous professional development to enhance cultural competence and clinical knowledge.
Adhere to hospital and departmental policies related to confidentiality, patient rights, infection control, and safety.
Decision Making :
Exercises independent professional judgment in resolving complex psychosocial issues, within the framework of hospital policies and social work best practices.
Working Relationships :
Collaborates directly with patients, families, physicians, nurses, and interdisciplinary teams.
Coordinates with hospital administration, community agencies, and external providers.
Engages frequently with staff at the Director level or above .
Required Skills & Experience :
Experience working with a clinical or hospital-based patient population.
Strong interpersonal, communication, and advocacy skills.
Computer literacy, including EMR systems.
Ability to manage multiple cases and adapt to a fast-paced environment.
Preferred Skills & Experience :
Previous experience in a hospital or community health agency .
Multilingual fluency (English + other languages preferred).
Education Requirements :
Master of Social Work (MSW) or closely related degree from an accredited program (required).
Certifications & Licensure :
