Doctor of Pharmacy (PharmD) degree, active Pharmacist license, and minimum 1+ years Pharmacist experience required. Applicants who do not meet these qualifications will not be considered.
A respected hospital in the Milford area is seeking a dedicated Oncology Clinical Pharmacist to join their team on a full-time, direct hire basis. This role is integral to delivering exceptional care to cancer patients across inpatient and outpatient settings.
Type : Direct Hire Full Time
Schedule : Day shift, MondayFriday
Start Date : ASAP
Key Responsibilities
Collaborate with oncologists and the healthcare team to ensure accurate treatment plans and optimal supportive care.
Provide expert drug information and review oncology care plans to align with current national guidelines.
Round with physicians, oncology fellows, and nurse practitioners to remend medication therapy adjustments.
Deliver patient-specific medication education for oral oncolytics and intravenous chemotherapy.
Precept pharmacy residents and students, and conduct educational programs for staff.
Participate in oncology service meetings to improve the safety, quality, and cost-effectiveness of cancer care.
Qualifications
Doctor of Pharmacy (PharmD) required.
Current Delaware Pharmacist license (or eligibility).
Board Certified Oncology Pharmacist (BCOP) preferred.
pletion of PGY1 residency plus at least 3 years of oncology practice, or PGY2 oncology residency, or at least 5 years of oncology practice with BPS board certification.
Minimum 3 years of hospital clinical pharmacy experience.
Strong knowledge of evidence-based oncology guidelines and excellent munication skills.
