This position is eligible for a $2,000 sign on bonus, eligible to be paid out after 90-days of satisfactory employment!
SUMMARY :
The Community Health Representative (CHR) promotes health and wellness in the Prairie Island Indian Community by visiting hospitalized patients, assessing and assisting client home wellness using sound judgement and setting priorities, providing transportation and med pick-up, promoting preventative training, workshops and education in-services, teaching, and topics shared in monthly newsletter under the supervision of the Family Health Director. Assists Family Health Department team members as assigned.
DUTIES AND RESPONSIBLITIES :
The following duties and responsibilities are not meant to be all inclusive and may be adjusted to meet business needs.
- Consult with medical staff on behalf of client to establish care protocols and review duties of CHR as needed
- Provide transportation for eligible clients to routine, non-emergency medical and dental appointments who have no other means of transportation per transportation policy
- Visit community members who are hospitalized in the surrounding area
- Participate in case plan for chronic / terminal patients and follows plan on a regularly scheduled basis
- Assess needs and make referrals based on weekly home visits from the continuum of care, newborn to geriatric to meet tribal member needs as time allows
- Serve as liaison between client population, service agencies and the medical providers
- Provide home health care services
- Provide or assist in demonstrations, conduct training sessions and community meetings in subjects such as safety, nutrition, environmental health and other areas of health concerns on a monthly basis
- Provide extensive health promotion / disease prevention information and instruction in self-help services that address chronic health issues such as articles in monthly newsletter or providing pamphlets and other forms of information.
- Work with health services staff to coordinate annual events such as Health Fair, 5K
- Pick-up and deliver prescriptions for community members, according to policy
- Enter Encounter Sheet data into the RPMS program and CHR Exports on a monthly basis
- Take vitals, create referrals, perform health screenings, follow-up as needed
- Complete online Indian Health CHR-specific trainings within 3 months
- Document client activity in RPMS as indicated
- Sanitize client eating and bathing areas in home, or tribal sites
- Assist delivering food, providing eating assistance, and supervision of elder or disabled clients at events
- Respond to Tribal medical emergencies if possible
- Prepares and submits monthly reports to department director
- Performs other duties as assigned
QUALIFICATIONS : Required
High School Diploma or equivalentMust obtain CPR / First Aid certification within first 3 months of employmentMinimum of six (6) months of home healthcare or nursing assistant experienceExperienced homemaking skills, including cultural practices of homecareKnowledge of the Tribal practices and normsGood organizational skillsExcellent written and verbal communication skillsStrong computer skills Microsoft Office (Word, Excel and Outlook)Ability to work independently and effectively with minimal supervisionAbility to work well within the existing team and government structureSensitive to Native American CultureMust pass drug screen (UA) and a federal criminal background check in compliance with the Indian Child Protection and Family Violence Prevention Act and the Adam Walsh Child Protection and Safety ActValid drivers license and proof of insurabilityPreferred
Certified Nursing Assistant, First Responder, or current student or graduate of a nursing program or related fieldParticipation in or coursework in Community Health Services or Environmental Health TrainingExperience working with elderly populationPHYSICAL DEMANDS :
