Join Our Team as a Home Care Certified Occupational Therapy Assistant (COTA)

At Carle Health, we’re looking for a dedicated Home Care Certified Occupational Therapy Assistant (COTA) to join our dynamic team. In this role, you’ll collaborate with healthcare professionals to create and implement treatment plans for patients of all ages, guiding them in their daily activities and helping them thrive.

Your Responsibilities

As a COTA, you will :

• Review patient orders and follow care plans developed by the Occupational Therapist and Physician.
• Educate patients and their families, ensuring understanding and overcoming barriers to learning.
• Document patient visits comprehensively, providing clear plans for future care.
• Communicate effectively with the healthcare team, initiating case conferences as needed.
• Assess and document patient pain levels and adjust care plans accordingly.
• Anticipate supply needs to ensure patients have what they need between visits.
• Assist in discharge planning, coordinating with physicians and community resources.
• Guide patients in therapeutic activities to enhance their functional abilities.
• Instruct other health team personnel and family members in occupational therapy techniques.
• Assess home safety and recommend equipment to improve patient safety.

Licensed Occupational Therapy Assistant (OTA) in Illinois
