Shift : MondayFriday, 85, with possible weekend rotation and on call

Job Description :

Sunbelt Staffing is seeking an experienced Home Health RN for a contract opportunity in Scottdale, PA. This role involves managing a field caseload, delivering skilled in-home care, and completing timely OASIS documentation. Ideal candidates are autonomous, organized, and confident with electronic charting.

Key Responsibilities :

Perform admissions, routine visits, and discharges

Complete accurate OASIS documentation using Homecare Homebase (HCHB)

Provide skilled nursing care, including wound care, IV administration, and patient education

Collaborate with physicians, case managers, and therapy staff

Support patients and families with care planning and disease management

Qualifications :

Active Pennsylvania RN license

2+ years of nursing experience, with at least 1 year in home health

Required : OASIS and Homecare Homebase (HCHB) proficiency

Current BLS certification

Reliable transportation, valid drivers license, and auto insurance

Able to meet all Joint Commission compliance standards
