Job Title : Advanced Practice Provider (APP) / Physician Assistant or Nurse Practitioner

Location : Summit County, OH (multiple clinic sites within 15 minutes of each other)

Salary / Pay Range : $110,000 $120,000 annually / $52.88 $57.69 / hr; Eligible for quarterly incentive bonuses (35% of salary) tied to productivity.

Reports To : Medical Director or Designee

The Advanced Practice Provider (Nurse Practitioner or Physician Assistant) will provide high-quality, holistic primary medical and behavioral health care to patients of all ages. Working under a collaborative agreement with a physician as mandated by Ohio law, the APP is responsible for the assessment, diagnosis, treatment, and coordination of care across the lifespan.

Key Responsibilities :

• Examine, diagnose, and treat acute and chronic conditions in patients of all ages.
• Perform wellness exams, including well-child, well-woman, and adult preventive care.
• Order, interpret, and act on laboratory and diagnostic imaging results.
• Develop and implement individualized treatment and care plans.
• Prescribe medications and therapeutic interventions within scope of practice.
• Deliver patient education focused on disease prevention, health maintenance, and care plan adherence.
• Provide age-appropriate and culturally competent care.
• Collaborate with interdisciplinary teams and specialists to ensure coordinated care.
• Refer patients to specialty services as needed and manage follow-up care.
• Participate in call rotation and provide weekend or extended coverage as assigned.
• Maintain accurate and timely documentation in compliance with regulations and standards.
• May support training or precepting of medical / nursing students.
