We're seeking a part-time passionate BC / BE Internal Medicine physician to join our team of experienced practitioners and specialists delivering exceptional patient care to older adults.

Schedule : Mondays 4 to 5 hours at Applewood Village and Tuesdays, 5 to 6 hours at Pines Village

Essential Functions :

• Provide comprehensive medical care and management for sub-acute residents, with occasional support for Assisted Living (AL), Long-Term Care (LTC), and Independent Living (IL) residents as needed.
• Conduct thorough geriatric assessments to evaluate physical, cognitive, functional, and other aspects of health, tailoring care plans to meet individual needs.
• Manage complex medical conditions commonly associated with aging, ensuring patient safety, comfort, and quality of life.
• Collaborate with interdisciplinary teams, including Nurse Practitioners, nurses, and allied health professionals, to develop and implement personalized care plans.
• Communicate proactively with families, addressing concerns and providing updates on patient progress.
• Educate patients, families, and caregivers on health issues, disease management strategies, medication management, and preventive health measures.
• Comply with regulatory requirements, documentation standards, and ethical principles specific to the nursing home environment.
• Handle admissions and quarterly reviews as part of ongoing patient management.
