POSITION SUMMARY
The Clinical Pharmacist is responsible for assessing patients, initiate and adjust medications, and monitor and treat disease states across multiple disciplinary fields.

ESSENTIAL SKILLS AND EXPERIENCE :
Graduate of an ACPE – accredited School of Pharmacy with a PharmD degreeCurrent Arizona State Board of Pharmacy License in good standingMinimum 5 years experienceValid Level One Fingerprint Clearance Card issued by the Arizona Department of Public Safety for all specialty behavioral health locationsCertification to perform Cardiopulmonary Resuscitation for Health Care Professionals (CPR) and AEDValid Arizona Driver’s LicensePOSITION REPONSIBILITIES
Advise on medication therapies for patientsEnsure therapies are safe, appropriate, and cost-effectiveEvaluate and order pertinent laboratory workRefill, monitor, and adjust medications as neededProvide education to patientsDiscuss adverse effects, disease progression, and potential lifestyle changes during visitEnsure that patient understands how to appropriately take / use medicationsCounsel patients on medications and adverse effectsClinical consultations with practitioners, recommending therapies and monitoring plans as neededAddress patient concernsIdentify any medical conditions not addressed by primary care teamRefer patient to appropriate available programsConsult with providers and nursing staffMedication reconciliationMedication therapy managementEnsure cost-effective medication is available and in useIdentify and provide resolution for instances where cost-effective medication is available to patientsEducation and communication with staff and providers on cost-effective medication and medical therapies when availableContribute to effective department operationsDocument financial impact of clinical activitiesMonitor performanceAssist in writing and implementing policies and proceduresPrepare reports as requestedTravel to multiple sites to assist across all Adelante pharmaciesOther duties as requested or assignedNONESSENTIAL SKILLS AND EXPERIENCE :
