Lead Care Manager

A company is looking for a Case Manager.

Key Responsibilities

Assess member needs and oversee the development of client care plans

Connect clients to necessary social services and advocate on their behalf

Maintain documentation and evaluate client progress while ensuring program compliance

Required Qualifications

2+ years of experience as a care manager, care navigator, or community health worker

Experience working in a remote setting

Working knowledge of government and community resources related to social determinants of health

General computer skills and familiarity with Google Workspace and MS Office

Bilingual (English / Spanish) is a plus
