Physician - Director for Home-Based Primary Care (HBPC)

This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific eligibility requirements per VHA policy and submit your EDRP application within four months of appointment. Program Approval, award amount (up to $200,000) & eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after review of the EDRP application. Former EDRP participants ineligible to apply.

This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until December 15th, 2025. The initial cut-off date for referral of eligible applications will be October 29th, 2025, with subsequent cut-off dates every 2 weeks. Eligible applications received after that date will be accepted on an ongoing basis and qualified candidates will be considered as vacancies become available.

Work Schedule : 7 : 00am to 3 : 30pm, 7 : 30am to 4 : 00pm, or 8 : 00am to 4 : 30pm, Monday through Friday

Pay : Competitive salary, annual performance bonus, regular salary increases

The Home-Based Primary Care (HBPC) program provides cost-effective primary care services in the home setting including palliative care, rehabilitation disease management, and coordination of care. The HBPC program targets frail, chronically ill veterans who require care by interdisciplinary healthcare teams, continuity, coordination of care, and the integration of diverse services to cover a complex medical, social, rehabilitative, and behavioral health care needs. These veterans need comprehensive, longitudinal home care services, as they age, to maximize function, minimize institutionalization, and maintain quality of life.

The HBPC physician medical director is responsible for the overall medical care delivered by the HBPC team and will also be responsible for the overall medical care of patients with conditions requiring treatment in their specialty to include consultative, prescriptive, and ordering authority in the outpatient / ambulatory and or inpatient basic setting at the VA Southern NV Health Care System. The incumbent provides patient care directly and / or through the clinical oversite of trainees to include residents, fellows and / or medical students in the home setting. In addition to their role as clinician, the HBPC medical director will have administrative responsibilities for clinical leadership and direction of HBPC development and programs, as well demonstrating effective interface with the HBPC staff. The HBPC medical director will participate fully in all accreditation processes, reviews, and site visits.

The major duties and responsibilities include, but are not limited to :

• Lead and direct the development of HBPC program.
• Ensure HBPC program compliance with meeting accreditation standards, including but not limited to VHA and Joint Commission standards for home care, ensuring that primary care services are provided.
• Provide clinical leadership and clinical program management for the HBPC Program.
• Assuming primary medical responsibility for HBPC patients by identifying the patients', medical problems and defining the medical management of these problems.
• Providing primary medical care to HBPC patients in the home setting, including serving as a consulting physician for advanced practice nurses and serving as back-up for other HBPC primary care providers.
• Provision of geriatric and palliative care consultation in acute care, outpatient, and home health settings. Provide clear and complete direct and electronic communication with staff in all areas of the medical center and outside contacts to facilitate smooth patient transitions, quality patient care and high customer satisfaction.
