Hospice Nursing Job Details

Employment Type : Locum tenens (Travel Contract)

Location : Paducah, KY – serving Fulton, Hickman, and Carlisle Counties (Kentucky nursing jobs)

Setting : Hospice Case Management

Duration : 13 weeks (Start : 10 / 6 / 2025 – End : 1 / 4 / 2026)

Shift : Monday – Friday, 8 : 00am – 5 : 00pm (8-hour days, 40 hours per week)

On-call : Backup on-call 1–2 days per week

Weekly Estimated Pay : $1850–$1890 per week

Registered Nurse Qualifications

Active Registered Nurse (RN) license in Kentucky

Minimum 1 year of hospice nursing experience (hospice RN jobs)

Proficiency with Homecare Homebase (HCHB) electronic charting

Strong communication and organizational skills

Ability to work independently and travel within assigned territory

RN Case Manager Responsibilities

Provide compassionate hospice case management for patients and families

Coordinate and deliver individualized patient care plans

Collaborate with interdisciplinary healthcare team members to ensure quality care

Document patient assessments and care using HCHB

Participate in backup on-call rotation as scheduled
