Population Health Medical Assistant
The Population Health Medical Assistant (Pop Health MA) plays a key role in improving patient outcomes by supporting proactive, preventive, and chronic care management efforts. This role combines the clinical skills of a Medical Assistant with responsibilities in care coordination, outreach, and population health initiatives. The Pop Health MA works collaboratively with providers, nurses, and care teams to identify and close care gaps, support quality measures, and ensure patients receive timely, comprehensive care.
Responsibilities :
- Utilize MA training and experience to support population health initiatives while maintaining readiness to assist in direct patient care as needed.
- Review population health reports, registries, and care gap lists to identify patients in need of screenings, labs, immunizations, and follow-up care.
- Conduct patient outreach via phone, portal, or mail to schedule preventive services and chronic care visits.
- Document outreach, care gap closure, and patient interactions accurately in the electronic health record (EHR).
- Support providers and clinical teams in meeting quality metrics, value-based care contracts, and compliance requirements.
- Provide patient education on preventive health, chronic disease management, and available community resources.
- Coordinate with providers, specialists, and care managers to support continuity of care.
- Perform traditional MA duties as needed, including vital signs, rooming patients, administering immunizations, and assisting with procedures.
- Maintain confidentiality and adhere to HIPAA and organizational policies.
Qualifications :
Completion of an accredited Medical Assistant program and current MA certification / registration preferred.Minimum 12 years of experience as a Medical Assistant in a clinical setting required.Strong knowledge of preventive care, chronic disease management, and population health concepts preferred.Excellent communication, organizational, and time management skills.Proficient with EHR systems and comfortable working with reports, registries, and data tracking.Ability to work independently and as part of a multidisciplinary team.Work Environment :
Combination of computer-based work, patient outreach, and occasional direct patient care.Regular collaboration with providers, care coordinators, and nursing staff.Benefits :
