Home Health Physical Therapist – Antigo, WI

Assignment Length : 13 Weeks

Start Date : ASAP

Shift : Monday–Friday | 8 : 00 AM – 4 : 30 PM (8 : 00–5 : 30 PM schedule noted by facility)

Hours : Guaranteed 37 hours / week (flex up to 40)

Shift Length : 8-hour days

Low Census : Up to 36 unbillable hours allowed

Job Description

Aspirus at Home is seeking a motivated Home Health Physical Therapist to join their team in Antigo, WI. This role provides care to primarily adult and geriatric populations in a home health setting. The PT will perform evaluations, develop treatment plans, and deliver skilled therapy per established standards.

Schedule & Requirements

  • Monday–Friday
  • Occasional travel to other branch locations (mileage reimbursed at federal rate)
  • Rotate one Saturday every 6–8 weeks (weekday off given when worked)
  • No call
  • No holidays

    • Responsibilities

  • Perform evaluations and develop patient-centered care plans
  • Deliver skilled therapy services to home health patients
  • Documentation in NetSmart EMR using company laptop
  • Maintain productivity of avg. 4.5 visits / day
  • Provide treatment for orthopedic, neurological, and functional mobility diagnoses

    • Required Experience

  • Previous Home Health experience required
  • Experience with adult / geriatric patient population
  • Strong problem-solving, adaptability, and communication skills

    • Education & Licensure

  • Bachelor’s (minimum) or Master's degree in Physical Therapy
  • Active PT License – Wisconsin
  • AHA BLS required
  • NPI required (must be in process if not already issued)

    • 50+ Miles radius rule applies
