As an RN Hospice Case Manager, you will develop and implement personalized nursing care plans while maintaining accurate medical records. You will ensure patient safety, recognize when to escalate concerns, and collaborate closely with interdisciplinary team members.
Essential Functions
- Create collaborative care plans based on medical and nursing diagnoses for patients ranging from newborns to geriatrics.
- Deliver skilled nursing services aligned with the established care plan.
- Complete accurate OASIS assessments as required.
- Work closely with physicians, patients, families, and other disciplines to address care needs and update plans accordingly.
- Supervise Home Health Aide services following agency policies.
- Participate in on-call rotations to provide 24-hour nursing availability for hospice, homecare, and private duty patients.
- Maintain timely and precise documentation in accordance with agency standards.
- Foster positive relationships with patients, families, colleagues, physicians, and referral sources.
- Promote patient independence by involving patients and families in discussions about medical issues, testing, medications, and self-care skills.
- Observe patients' physical, mental, emotional, and spiritual conditions and collaborate with team members as appropriate.
- Adhere to HIPAA privacy and security regulations regarding Protected Health Information (PHI).
Minimum Knowledge and Skills

Graduate of an accredited nursing program.Several years of clinical nursing experience.Preferred : Bachelor of Science in Nursing (BSN).Preferred : Previous hospice and / or home health experience.Preferred : Certification as a Hospice and Palliative Care Nurse.Licenses and Certifications
