Job Description :
The Outpatient Behavioral Health Advanced Practice Provider is responsible for delivering comprehensive mental health care in an outpatient setting, including diagnosing and treating various psychological conditions. This role involves conducting assessments, developing and implementing individualized treatment plans.
Behavioral Health services are designed around a collaborative multi-disciplinary team that includes psychiatrically trained Physicians, Advanced Practice Providers, Social Workers, Peer Recovery Support Specialists, MH technicians, a Clinical Pharmacist, and nursing staff that are experienced and committed to excellent, highly collaborative care.
Patient Care & Clinical Responsibilities :
- Coordinate multidisciplinary care for all patients to ensure high-quality, optimized outcomes.
- Conduct comprehensive psychiatric evaluations and mental health assessments to determine patient needs and treatment goals.
- Diagnose mental health conditions and develop individualized treatment plans, including medication management and other interventions.
- Provide evidence-based, compassionate care to improve patient outcomes and enhance patient satisfaction.
- Educate patients and families about care plans, treatments, and health management.
- Make appropriate referrals to support comprehensive and continuous care.
Collaboration & Communication :
Maintain a collaborative environment with providers, clinic staff, outpatient clinic leaders, and behavioral health (BH) physician and APP leaders.Communicate effectively across disciplines to support integrated care delivery.Quality Improvement & Compliance :
Participate actively in Performance Improvement Programs and Peer Review committee meetings.Monitor and ensure compliance with care management guidelines established by JPS Health Network, Acclaim Physician Group, and relevant accreditation agencies.Academic, Educational & Research Support :
Support and engage in quality improvement initiatives, academic programs, educational activities, and research efforts.Duties and Essential Job Functions :
Coordinates the multidisciplinary care delivered to all patients, to optimize patient care.Reviews patient medical charts to diagnose, treat, and provide care management and education to patients and families.Makes appropriate patient referrals as necessary.Monitors guidelines for the management of care and for ensuring the services are delivered in compliance with the guidelines established by JPS Health Network and Acclaim Physician Group.Provides evidence-based and compassionate care to all patients as assigned to improve patient outcomes and increase patient satisfaction scores. Demonstrates patient centered and patient valued behavior.Maintains a collaborative environment with other referring physicians, residents, clinical staff, and support staff to provide quality patient care.Supports academic, educational and research endeavors.Attends Performance Improvement Programs and Peer Review committee meetings; and is an active participant in the process.Partners and communicates with acumen to build a positive brand and patient / customer experience for those who encounter JPS Health Network and Acclaim Physician Group. This would include health network staff, vendors, outside partners, patients, stakeholders, and local community leaders.Serves as a resource to the community.Performs other job-related duties as assigned.Qualifications :
