Key Responsibilities

Deliver telephonic care management for patients enrolled in Value-Based Care Programs

Support quality gap closure and coordinate care with providers and the interdisciplinary team

Communicate patient health updates and treatment plans to caregivers and providers

Required Qualifications, Training, and Education

Active, unrestricted compact LPN license

2+ years of nursing experience in various care settings

Knowledge of pathophysiology and treatment protocols for common health diagnoses

Proficient computer skills and ability to adapt to technology platforms

Ability to work independently in a remote environment
