Your Role as a Pediatric Registered Nurse - Case Manager :

As a Case Manager, you'll play a vital role in coordinating and overseeing the care of pediatric patients while serving as a key advocate for families. Your responsibilities will include :

Patient Assessments & Care Planning

Conduct comprehensive assessments, develop individualized care plans, and adjust treatments as needed.

Care Coordination & Advocacy

Collaborate with physicians, therapists, and caregivers to ensure seamless, high-quality patient care.

Family Education & Support

Teach families about treatment plans, disease management, and best practices for home care.

Clinical Nursing Care

Provide direct care, including medication administration, wound care, tracheostomy / ventilator management, and emergency interventions.

Monitoring & Documentation

Track patient progress, recognize changes in condition, and ensure accurate documentation within the client's record.

Leadership & Team Collaboration

Work effectively within a multidisciplinary team, demonstrating flexibility and problem-solving skills in various healthcare situations.

Compliance & Quality Assurance

Uphold Elara Caring's mission, philosophy, and policies to maintain the highest standards of care.

What You Need to Succeed :

At least 1+ year of RN experience with Pediatric experience

Physical Requirements - Ability to sit, stand, bend, lift, and move intermittently, with the capability to lift 50-100 lbs.

Licensure

Active and unrestricted RN license in the state of practice.

Transportation

Reliable vehicle, valid driver's license, and current auto insurance (per state laws).

Skills & Attributes

Strong clinical judgment, problem-solving abilities, and the ability to communicate effectively with families and healthcare teams.
