LPN - Resource Coordinator

Full Time LPN - Resource Coordinator     The Resource Coordinator will assist under / uninsured individuals in the community diagnosed with Opioid Use Disorder (OUD) in receiving medication assisted treatment with counseling and behavioral therapies in accordance with the LaSOR Opioid State Targeted Response (STR) Grant, funded by the Substance Abuse Mental Health Services Administration (SAMHSA).     Essential Duties & Responsibilities :

• Meet with clients to obtain information for the Government Performance and Results Act.
• Track all clients in the LaSOR program.
• Log / report data per requirements of LaSOR Program Manager.
• Participate in program recruitment and referrals.
• Participate in the LaSOR monitoring and reporting process.
• Oversee the LaSOR Care Coordination process among participating agencies.
• Ensure multiple needs of clients are addressed and appropriate referrals are made. Assist patients with applying for benefits, SNAP, disability, transportation, etc.
• Work toward identification, engagement, and retention of clients.
• Maintain working knowledge of array of services available in surrounding areas.
• Coordinate resources through multiple points of entry and delivery systems for the benefit of the client’s wellbeing.
• Verify insurance status of each client and assess uninsured patients’ eligibility for Medicaid. If the patient is Medicaid eligible, he or she will be required to apply for Medicaid.
• Ability to handle stressful situations and interact with others.
• Must be present during working hours at the office for in-person meetings and access to a computer without violating company policy.
• Complete NARCAN training with clients.
• Other duties, as assigned.
