Job description$25–$30 / hour Performance bonus : $2,500 for every 50 enrollments in a single month Mileage reimbursement Training, marketing materials, and equipment provided Outreach or enrollment experience, ideally with CalAIM or similar programs Go-getter mindset with ability to work independently in the field Valid driver's license and comfort traveling to different locations CHW Certificate preferred (strong plus) Education : Bachelor's in Social Work, Psychology, or Sociology + 2 years' experience OR 4–5 years' direct experience in lieu of degree Paid Sick Leave (Medix provides paid sick leave according to state and local sick leave ordinances). Health Benefits / Dental / Vision (Medix offers 6 different health plans : 3 Major Medical Plans, 2 Fixed Indemnity Plans (Standard and Preferred), and 1 Minimum Essential Coverage (MEC) Plan. Eligibility for health benefits is based on verifying that an average of 30 hours per week during the first 4 weeks of the work assignment has been met. If you meet eligibility requirements and take action to enroll, you will be covered no earlier than 60 days into your assignment, depending on plan selection(s)). 401k (Eligible on the first 401k open enrollment date following 6 consecutive months on assignment. 401k Open Enrollment dates are 1 / 1, 4 / 1, 7 / 1, and 10 / 1). Job Summary
Our client is seeking a Community Health Worker responsible for bridging the gap between the community and health services. The role involves promoting health practices, ensuring access to healthcare resources, and advocating for community health needs.
What You'll Do
- Conduct outreach in the community to educate and enroll patients in CalAIM
- Manage a caseload of 25–50 clients, including assessments and referrals
- Provide housing navigation and case management support as needed
- Set up outreach tables at clinics, hospitals, and community events
- Build relationships with local facilities and community partners
