What We Are Looking For :

Job Title : Hospice Aide Specialist (Days) - Kaplan Family Hospice Residence (KFHR)

Schedule : Full-time position consisting of three (3) 12.5-hour shifts per week, with an every-other-weekend rotation required.

Shift : 7 : 00 am - 7 : 30 pm

About this role :

As a Hospice Aide Specialist, you'll make a meaningful difference in the lives of our patients and their families. This role provides direct, hands-on care in alignment with the patient's plan of care while supporting their comfort, dignity, and well-being. You'll work closely with our nursing staff and the interdisciplinary team to ensure each patient receives exceptional, personalized support.

What You'll Do :

• Assist patients with daily personal care needs including bathing, grooming, dressing, toileting, and mobility.
• Monitor and report patient observations and changes in condition to the RN.
• Help maintain a clean, safe, and healthy environment.
• Follow proper infection-control and safety procedures.
• Offer companionship through conversation, activities, and emotional support.
• Provide simple nursing support, vital sign checks, and emergency assistance as needed.
• Support caregivers by offering respite and responding promptly to patient / family needs.
• Work collaboratively as part of the hospice interdisciplinary care team.
