Social Worker, Care Management
Location :  Santa Barbara, CA (Hybrid)
Classification :  Exempt
Compensation :  $76,000–$111,000 annually, depending on experience
Overview :
Our client is seeking a compassionate and resourceful Social Worker to join their Care Management team supporting members in a Dual Special Needs Program (D-SNP). This role focuses on helping members navigate psychosocial and social-determinant needs while coordinating services across medical, behavioral health, and community programs. The ideal candidate is relationship-driven, detail-oriented, and passionate about improving access to care for vulnerable populations.
Responsibilities :
- Conduct person-centered psychosocial assessments and develop individualized care plans in collaboration with the interdisciplinary care team.
- Coordinate services across healthcare, behavioral health, long-term services and supports (LTSS), and community-based programs.
- Facilitate smooth care transitions between settings (hospital, skilled nursing, home), ensuring members receive needed follow-up and support.
- Connect members and caregivers with community resources such as IHSS, housing assistance, transportation, and food programs.
- Deliver brief supportive interventions using motivational engagement and problem-solving strategies.
- Maintain accurate, timely documentation of assessments, care plans, referrals, and outcomes.
- Participate in team meetings, case rounds, and quality improvement initiatives to enhance member outcomes.
- Uphold confidentiality, ethical practice, and compliance with HIPAA and program standards.
Qualifications :
Master’s degree in Social Work, Clinical Psychology, or Psychology required.Minimum of 5 years of relevant clinical experience in care management, behavioral health, or medical social work.Experience supporting medically complex, high-risk, or vulnerable adult populations.Knowledge of Medi-Cal, Medicare, and managed-care systems.Strong communication and collaboration skills with the ability to build trust with members and families.Excellent time management and documentation abilities.Preferred :
Licensed Clinical Social Worker (LCSW) or equivalent license.Bilingual in Spanish.Location & Work Environment :
Hybrid role with local presence required for home visits, team meetings, and community outreach. Regular business hours apply.
