About The Position

Part-Time & Full-Time Nurse Practitioner – Geriatric Primary Care

Location : New York (On-Site at Assisted Living Facilities)

Compensation : $60,000 – $120,000 per year

Are you passionate about making a difference in the lives of the geriatric population? We are seeking a compassionate and skilled Nurse Practitioner to provide comprehensive primary care services to residents of assisted living facilities. The ideal candidate will have experience working with older adults and / or long-term care and be dedicated to delivering high-quality, patient-centered care. You will conduct thorough health assessments, develop individualized care plans, manage chronic and acute conditions, and collaborate closely with interdisciplinary teams to optimize patient outcomes. This role offers both part-time and full-time opportunities, with no nights or weekends—perfect for maintaining a healthy work-life balance.

Job Requirements

2. Requirements

• Master’s degree in Nursing from an accredited Nurse Practitioner program
• Active, unrestricted New York State Nurse Practitioner license
• Valid DEA permit to prescribe medications
• Eligibility to provide reimbursable services and maintain provider status with Medicare and Medicaid
• Minimum of 3 years clinical experience, preferably in geriatrics or long-term care
• Experience with eClinicalWorks (eCW) electronic health records preferred
