Lakeview Center provides comprehensive behavioral health care to adults and children with mental illnesses, drug and alcohol dependencies and intellectual disabilities. Across Northwest Florida, our services range from residential treatment to outpatient counseling, psychiatry, trauma care, treatments for substance misuse and 24 / 7 support for those with serious mental illnesses. Adult Psychiatric Support provides supportive services including, but not limited to, crisis counseling, short-term and center-based case management, case coordination, and medication refill assistance. Assessments and referrals to other programs are made based on client need and recommendations from LCI psychiatrists and APRNs.

Overview

The purpose of case management is to assist clients in gaining access to needed medical, social, educational, and other services.

The primary goal of case management is to optimize the functioning of individuals who have complex needs by coordinating the provision of quality treatment and support services in the most efficient and effective manner. The desired outcomes for persons using the service system are self-sufficiency, and satisfaction in the living, learning, work and social environments of their choice.

Salary Range for this position is $20.2255 to $21.8558 per hour.
